Track client progress over time using psychometric score charts in Tacklit. View trends across sessions for any assessment type to support clinical decision-making.

Tacklit collects psychometric assessment data throughout the care journey, and the Scores Over Time chart gives you a visual way to track how a client is progressing across sessions. This is useful for identifying trends, spotting early signs of deterioration, and sharing measurable outcomes with clients or their care team.

Where to find it

Open the client profile and navigate to the Assessments tab. This tab lists all psychometric assessments that have been completed for the client, whether submitted through the client portal, completed during a session, or entered manually by a clinician.

Viewing scores over time

1. On the Assessments tab, use the Assessment Type dropdown to select the measure you want to review (e.g., PHQ-9, GAD-7, K10, DASS-21).

2. Toggle the Chart view. This switches from the list of individual assessment results to a line chart showing scores plotted over time.

3. The chart displays each completed assessment as a data point, with the date on the horizontal axis and the score on the vertical axis. This makes it easy to see whether scores are trending up, down, or staying flat.

How to use this clinically

Track treatment progress. If you are using a standardised measure like the PHQ-9 at regular intervals (e.g., every 4 sessions), the chart gives you an at-a-glance view of whether your intervention is having an effect. A downward trend on a depression measure, for example, indicates improvement.

Detect early deterioration. A sudden spike in scores between sessions may indicate a change in circumstances or an escalation in symptoms that warrants clinical attention. The visual format makes these changes easier to spot than scanning a list of numbers.

Support supervision discussions. When reviewing a case with a supervisor, the chart provides an objective snapshot of the client's trajectory. This can inform decisions about treatment approach, session frequency, or whether a referral is needed.

Share with clients. Some practitioners find it helpful to show clients their score trends during sessions. Seeing measurable progress can reinforce engagement with treatment. You can also include assessment data in reports sent to the client or their support team using the Letters and Reports feature.

Supported assessment types

The chart works with any psychometric assessment configured in your service, including standard measures like PHQ-9, GAD-7, K10, DASS-21, and any custom assessments your service has set up. As long as the assessment has been completed more than once for a client, the chart will display the trend.

Tips

For the most useful trend data, administer the same assessment at regular intervals throughout the episode of care. Consistent timing (e.g., every 4th session or monthly) makes the chart more meaningful for clinical interpretation.

If you are using automated forms, you can configure assessments to be sent to clients at set intervals through the automation engine, so scores are collected consistently without manual effort.
